Job Description
Tucker House Nursing and Rehab -

  • Assist in the development and implementation of client care plans

  • Maintain accurate and up-to-date client records and case files

  • Conduct preliminary client interviews and assessments

  • Coordinate with other service providers and agencies to ensure comprehensive care

  • Facilitate communication between clients and social workers

  • Provide emotional support and counseling to clients under supervision

  • Help clients access community resources such as housing, healthcare, and financial assistance

  • Assist in organizing and conducting support group sessions

  • Monitor client progress and report any changes to the supervising social worker

  • Participate in team meetings and case conferences

  • Perform administrative tasks such as scheduling appointments and managing correspondence

  • Assist in crisis intervention situations as needed
