Job Summary for Behavioral Health Case Manager:
To assist individuals in gaining access to needed medical, social, educational and other services. The primary goal is to optimize the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner.
- Assesses the client’s and the family’s needs and develops a service plan that will improve the child’s overall level of functioning.
- Involves client, family and appropriate collateral providers in service planning and decision making as appropriate.
- Links the family with specified services and resources identified in the service plan.
Requirements:Education and Experience Requirements for Behavioral Health Case Manager:
- Bachelor’s degree in the field of social work, criminal justice, psychology or special education is required.
- A minimum of one year’s experience working with emotionally disturbed children and their families is needed.
- One year of previous Targeted Case Management experience.
- Good computer skills, and adapting to electronic health record changes.
- Reliable transportation with liability insurance, a FL driver’s license and good driving record are also required.
Preferred Education and Experience for Behavioral Health Case Manager:
- Current Targeted Case Manager certification preferred; if not certified, must receive appropriate training and attain the certification within three months of hire.
- Bilingual (Spanish)
