POSITION TITLE: Behavioral Health Clinician, Road to Resilience

REPORTS TO: Behavioral Health Manager, Perinatal Mental Heath and SUD
SCHEDULE: 40 hours per week at West Sacramento/Woodland clinics as well as in-home and in-community services. Some evenings shifts may be required. Travel between CommuniCare+OLE facilities required. Ability to meet clients in-home and in-community required.
LOCATION: West Sacramento, CA and Woodland, CA.
PAY RANGE:
- ASW/APCC/AMFT: $71,760 to $75,337.60 annually
- LCSW/LPCC/LMFT: $81,473.60 to $99,590.40 annually

JOB SUMMARY : The Behavioral Health Clinician reports directly to the Behavioral Health Perinatal Manager. The position will provide in-home/in-community and in-clinic engagement, service navigation, and individual and family counseling to Pregnant/Parenting clients enrolled in early intervention in-home visiting and support programs.
EDUCATION, EXPERIENCE, TRAINING
- ASW/LCSW, AMFT/LMFT, APCC/LPCC required. Must be in good standing with the Board of Behavioral Science
- 1 year minimum experience working with clients with perinatal mood and anxiety disorders, involved in addiction treatment and/or involved with Child Welfare Services preferred.
- Bilingual English/Spanish required

B. DUTIES AND RESPONSIBILITIES

1. | Engages clients in appropriate services, utilizing effective rapport building and motivational interviewing skills. |
2. | Conducts screenings and intake assessments for clients utilizing appropriate assessment tools. |
3. | Works with clients to development client centered treatment plans by establish long- and short-term goals and action steps to achieve goals. |
4. | Provides individual, group, couples, and family counseling and psychoeducation to clients in-home, in-clinic and in-community through the Road to Resilience Grant. |
5. | Supports client success and stabilization in treatment services by providing case-management, referral connection, monitoring progress and on-going engagement efforts. |
6. | Completes all client information and documentation in a timely manner according to CCHC, First 5, and OCAP guidelines and standards, including required pre/post screening and data collection tools. |
7. | Collaborates with multidisciplinary team to identify best practices for services provided. |
8. | Responsible for detecting emotional, psychological, and physical danger signals with a non-judgmental approach and offering feedback and support to clients. |
9. | Provide triage, crisis response, and support services to medical patients experiencing mental health symptoms, collaborate and communicate with medical staff regarding care coordination. |
10. | Responsible for communicating with and case coordination with referring agencies and other collaboratives including but not limited to First 5 Yolo County, Yolo County Children’s Alliance, Yolo Crisis Nursery, Yolo County Child Welfare Services, and Healthy Families Yolo County. |
11. | Attend collaborative with multidisciplinary team meetings with internal and external providers. |
12. | Responsible for remaining within the ethical standards established in the field, including those of confidentiality. |
13. | Responsible for explaining and enforcing all rules and regulations associated with clinic treatment policies. |
14. | Compile and report statistics as required. |
15. | Attend clinical supervision and weekly staff meetings. Attend training/conferences as arranged. |
16. | Responsible for maintaining the standards as listed in the CommuniCare Health Centers Personnel Policies and Procedures Manual, and any other standards which may apply to this position. |
17. | Responsible for maintaining current license/registration in good standing, submit all training certifications and CEUs to Human Resources, must attend ASAM Training 1x per year. |
18. | Other duties as assigned. |
