Job Type: Full-time, 40 hours per week

Job Description:

Transition coaches provide community-based, face-to-face interventions to individuals according to the principles and approaches of the Critical Time Intervention (CTI) evidence-based practice to support successful transitions in care. CTI is an intensive case management model designed to assist older youths and adults with behavioral health challenges to manage their transitional needs adequately. CTI promotes a focus on recovery and bridges the gap between institutional living and community services.


Requirements:

Key Responsibilities:

  • Provide CTI interventions for an assigned group of clients including coordinating and monitoring the activities of a documented transition plan.
  • Utilizing a person-centered approach, develop, implement, evaluate, and revise goals and plans in partnership with participants.
  • Provide individual skill-building to promote independent living and system management.
  • Ensure immediate changes are made in transition plans as clients’ needs change.
  • Consult with community agencies and natural support to ensure coordination in the service process.
  • Document service provision with individuals.
  • Participate in weekly organizational meetings.
  • Maintain the fidelity of the Critical Time Intervention model through each phase of transition.
  • Attending, completing, and implementing CTI training and methodology.
  • Conduct brief, evidence-based, and developmentally appropriate screening and conduct or arrange for more detailed assessments when indicated.
  • Provide services that are relevant to the culture of the consumer and family. This includes Identifying and addressing disparities in healthcare access and quality adapting services to language preferences and cultural norms and promoting diversity among the providers working on inter-professional teams.

Experience:

  • A bachelor’s degree in a related human service field
  • At least 2 years of experience working with behavioral health, developmentally delayed, incarcerated, and/or substance use populations – either older youth or adults.
  • Additional certifications or credentials are a plus.

OR

  • A high school diploma or GED
  • At least 4 years of experience working with behavioral health, developmentally delayed, incarcerated, and/or substance use populations – either older youth or adults.
  • A Certified Recovery Support Worker (CRSW) Certification through a program that is endorsed by the State of New Hampshire.
  • Additional certifications or credentials are a plus!

  • Ability to work independently, visit clients in the community, and respond appropriately to unexpected situations.
  • Ability to work constructively with clients facing a range of challenges stemming from behavioral health disorders, substance use, incarceration, short and long-term institutionalization, trauma, etc.
  • Ability to be even-tempered and flexible; establish rapport, rapidly develop, and maintain effective working relationships with diverse individuals, including healthcare consumers, family members, and other providers.
  • Proactively prioritize initiatives and can multitask. Ability to accept constructive feedback and initiate appropriate actions to correct situations.
  • Ability to function effectively as a member of an inter-professional team that includes behavioral health and primary care providers, consumers, and family members.
  • Ability to assess and continually improve the services delivered as an individual provider and as an inter-professional team.
  • Ability to acquire and maintain skills to develop, process, and input material on a computer using a variety of software packages, including Microsoft Office products.

Qualifications:

  • Demonstrated knowledge of social and human service providers, public benefits systems, and referral processes
  • Experience providing case management and individual assessments to clients.
  • Documented completion of training in any of these areas a plus: Critical Time Intervention (CTI); Stages of Change Training; Motivational Interviewing; Ethics; Cultural Competence; Person-Centered Planning; Suicidal Prevention
  • Current unrestricted driver's license and current personal vehicle insurance.
