Job Summary:


The Behavioral Health Intake Clinician provides services in an evidence based, trauma informed, culturally competent, and co-occurring competent manner. This position works with clients to provide through, consistent, and reliable assessment, individual psychotherapy, coordination of care, discharge planning, and case management services as necessary. Collaborates with internal and external colleagues, client collateral contacts and outside resources as needed to complete the essential job functions. Participates in clinical and administrative meetings.


Essential Job Functions:



  • Provides trauma-informed behavioral health services to individuals diagnosed with mental health and substance use disorders. Depending on clinician's scope of service, may include adults and/or children.

  • Facilitates complete and accurate screening and assessment of referred clients to ensure thorough and accurate client history is collected at the onset of treatment. The information initiates the correct and appropriate diagnosis to receive treatment services.

  • Maintains confidentiality of records relating to patients' treatment.

  • Connects patients with community and other resources to meet social needs.

  • Facilitates community-based case management that may occur outside of the office setting.

  • Collaborates with other providers and multi-disciplinary team to gather collateral information and coordinate care.

  • Guides clients in the development of skills or strategies for dealing with their problems using evidence-based treatment models.

  • Develops and implements individualized treatment plans.

  • Prepares and maintains clinical documentation, including assessments, treatment plans and progress reviews, admission and legal documents, and progress notes for the clinical record.

  • Assesses for suicide and other risks related to behavioral health, develops safety plans, and facilitates services to address complex client needs.


Education:



  • Maintains current Independently licensed to provide behavioral health services in the State of Wisconsin (LCSW, LPC, LMFT)

  • Master's Degree in Social Work or related field required.

  • Substance Abuse Counselor preferred.

  • Bi-annual BLS Certification required


Related Work Experience:



  • Two years in direct behavioral health clinical services preferred
