Job Description
Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.
Serenity Hospice Care is looking for compassionate and dedicated Social Worker to join our team! This position will be responsible for performing the social services section of the assessment process as well as provide on-going care to patients and their families.
Responsibilities:
- Managing and completing all psych-social assessments
- Educating the patient and family about services
- Managing financial information from patients and family
- With the interdisciplinary team, develop a plan of care.
- Assist the patient and family with difficulties (personal, financial, environmental)
- Collaborate with the team to manage any ongoing issues
- Assess and reassess ongoing social, emotional and financial factors for the patient and family
- Identify and promote community and family resources to assist with the patients plan of care
- Develop, prepare and maintain clinical documentation with accuracy, and on time according to the prescribed policies
Requirements:
- Hold a valid MSW from an accredited university.
- 1 Year of Hospice Experience
- Reliable Transportation & Willing to Travel
- Have working knowledge of territory
- Self-Managed; Independent
- Organized
What We Offer:
Monday - Friday Work Schedule (8:30 AM to 4:30 PM)
No Nights or Weekends required.
