Looking for a Hybrid Behavioral Case, Hybrid Remote and in Field

Hours: Monday – Friday, 8am-5pm, no weekends, no holidays
Pay: $28.00
Travel in County Required:

• Travel time is paid at the hourly wage
• Mileage is reimbursed at the IRS rate
• Must live in Granbury, Joshua, and Burleson

Hybrid Behavioral Case Manager Job Duties:

• Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
• Conducts comprehensive evaluation of Members using care management tools and information/data review
• Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs, and improve healthcare program/operational efficiency involving clinical issues.
• Determines medical necessity/appropriateness.
• Facilitates optimal outcomes.
• Identifies and follows through with continuous quality/compliance opportunities. May also include identification of aberrances and initiation of corrective action.
• Educates/empowers customers to ensure compliance, satisfaction, and promote patient advocacy.
• Coordinates and implements assigned care plan activities and monitors care plan progress
• Conducts multidisciplinary review to achieve optimal outcomes

Hybrid Behavioral Case Manager Requirements:

• Case Management experience required
• 2 years field case management experience preferred.
• Bilingual Preferred (Spanish).
• Bachelor’s Degree Required (Social Work or related field)
• Willing and able to travel using your own vehicle 50-75%

of their time to meet with members face to face in Granbury, Joshua, and Burleson and surrounding areas (Must have reliable
transportation and a drivers license)
Hybrid Behavioral Case Manager Preferred Qualifications:

• Long Term Care experience preferred
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
