Qualifications and requirements:
*Minimum Education*
Required Bachelor’s degree in Psychology, Sociology, or Social Work
Preferred Masters degree in Psychology, Sociology, or Social Work
Required Skills
*Minimum Work Experience*
Preferred - 5 years experience in Long Term Care case management or Social Work experience.
Five (5) years of supervisor experience in Long Term Care is preferred.
*Position Details:*
Establish a clear mission and vision for the department that aligns with the organizations strategic goals. Stay informed about current and emerging policies, practices, and technologies in healthcare. Ensure compliance with federal and state regulations and guidelines from accrediting agencies.
Communicate clearly and foster collaboration within Lifepoint and its facilities. Enhance services to achieve high-quality outcomes for customers. Perform Medicaid screenings for the NHP and assist patients with Safety Net applications as needed.
