How you can make an impact: As a Licensed Clinical Social Worker you will provide Behavioral Health consultation as part of an interdisciplinary team-based model of care. Our patients receive care in the OBGYN, Pediatrics, Internal Medicine, MAT, and Dental Departments. You will be a part of our growing mission-driven organization that thrives on collaboration, novel thinking, and transformation.
What you will do:
- Conduct brief/short-term/intermediate therapy for individuals and families
- Conduct biopsychosocial assessments and be familiar with other clinical assessments and use of the DSM in diagnosing and treatment
- Establish and formulate comprehensive treatment goals/plans
- Complete timely and accurate documentation in EMR
- Actively participate in case conferences with behavioral health, social work staff, and other clinical staff
- Make appropriate patient referrals for psych evaluations and medication management services
- Maintain and manage provider schedule and maintain active patient list (caseload)
- Attend monthly departmental meetings and supervision (if applicable)
- Educate patients in the health center and community in the nature of behavioral, mental, and social health determinants on mental health
Who you are and the expertise you bring:
- Compassion for helping patients.
- Must possess a current NJ License and or Certification: LCSW, LPC, LCADC
- Credentialling with Medicaid, Medicare, and other Insurance panels a plus
- Must have at least 3-5 years of experience working in a behavioral health setting providing individual and family counseling.
- Strong background and experience in working with diverse populations
- Experience working with children, adolescents, and adults
- Must maintain continuing education requirements
- Bilingual and fluent in Spanish is a plus
