Near North Health, a longstanding pillar of Chicago's healthcare landscape, is seeking a visionary and compassionate Licensed Clinical Social Worker (LCSW) to join our dedicated team. As Licensed Clinical Social Worker (LCSW), you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your ability to access and assist NNH clients/patients will further solidify our position as one of the largest providers of community-based primary care in the city.



Key Responsibilities:




  • Patient Advocacy: Assist patient in identifying/obtaining medical resources. Help patients plan for their home needs by assisting the patient with arranging at-home services.




  • Assessment: Conduct mental health assessments for referred NNH patients, through individual, group counseling and/or family counseling.





  • Treatment: Develop a service and or treatment plan for clients that are enrolled in domestic violence or clients receiving mental health counseling.




  • Training: Conduct community education awareness workshops regarding domestic violence with other NNH programs and other community agencies.


Qualifications


  • Perform on-going analysis of health center and community needs and staff deployment patterns in order to maximize the use of time and resources. Assist the Director in obtaining linkage agreements with organizations, agencies or other institutions that represent resources that might be deemed beneficial for our client population.

  • Make morning visits to Prentice, Michael Reese and Providence Hospitals for the purposes of checking on newborn deliveries and scheduling appointments with NNH medical services for follow-up care, as required.

  • Provides social work services at different health center sites of NNH as required




Education: Master’s Degree in Social Work from a School of Social Work approved by the Council on Social Work Education.



Required Licensure and/or Certification: Current State License, or work toward Licensure requirements



Minimum Experience: 3 years of experience in the field of social work (internships qualify). Must have the ability to: take direction/supervision; willingness to ask for clarity in ambiguous directives or situations; ability to establish and maintain professional working professional working relationships with staff and other professionals; ability to provide leadership in the areas of clinical relationships with staff and other professionals; ability to conceptualize and transmit knowledge and techniques in the resolution of problems.
