Job Description
Position Overview
Join our team as a Social Worker supporting the Southern Arizona VA Health Care System (SAVAHCS). In this role, you'll provide critical social work services to veterans, conducting assessments, coordinating care, and ensuring proper discharge planning while working as part of an interdisciplinary healthcare team.

Contract Details:
  • Location: Southern Arizona VA Health Care System (SAVAHCS) at 3601 S. 6th Ave. Tucson, AZ 85723
  • Position Type: Full-time (48 hours per week)
Key Responsibilities
  • Perform comprehensive biopsychosocial assessments
  • Conduct suicide risk, mental health, and crisis assessments and interventions
  • Document safety plans and care plans in patient medical records
  • Provide information and assist with Advanced Directives completion
  • Coordinate and facilitate family meetings and interdisciplinary team meetings
  • Conduct goals of care conversations and care planning
  • Assess and address cases of abuse or neglect, consulting with Adult Protective Services as needed
  • Manage discharge planning, including referrals to rehabilitation, assisted living, and other care facilities
  • Coordinate home health resources and support services for veterans
  • Provide assistance with housing resources for homeless veterans
  • Maintain patient confidentiality and comply with HIPAA regulations
  • Document all patient interactions in VA electronic medical records

Qualifications
Required:
  • Master of Social Work (MSW) from a nationally accredited school
  • Current, unrestricted Social Work license (LCSW)
  • Minimum of two years' experience in social work
  • Ability to obtain VA credentialing and privileges
  • Strong computer skills for electronic medical records
  • Ability to speak, read, and write English fluently
  • Must pass background check and drug screening
  • Required immunizations (TB, Rubella, Hepatitis B, Varicella, Pertussis, annual flu vaccine)
Preferred:
  • Previous VA or military healthcare experience
  • Experience with mental health crisis intervention
  • Knowledge of VA benefits and systems
  • Experience with interdisciplinary care teams
