The overarching goal of the Licensed Clinical Social Worker (LCSW) is to support and maximize the psychosocial functioning and adjustment of KidneySPA patients and their family/support systems. KidneySPA Social Workers provide services that improve social and emotional stresses resulting from the interacting physical, social, and psychological components of end stage renal disease (ESRD). KidneySPA Social Workers are an integral part of the interdisciplinary team who collaborate with other team members to help them in understanding the psychosocial factors that can impact the treatment outcomes of KidneySPA patients.
Some of the essential functions/responsibilities include:
•Assist patients with obtaining health care benefits and provide referrals for community based social services.
•Gather and report to the Billing Manager or Medical Director the status of the patient’s Medicare, and/or coinsurance status through the Social Security Office.
•Conduct financial evaluations of each patient prior to admission and monthly as necessary to determine indigency, primary and secondary insurance needs.
•Apply for Medicaid on all patients with no insurance or no secondary insurance or as needed and follow up on the outcomes.
•Identify and assist with resolutions of financial needs or issues that affect patient's quality of life and capacity to obtain ongoing treatment. Must work closely with the billing department.
•Appeal all medication denials based on insurance requirements.
•Provide necessary education regarding dialysis modalities to patients including his/her family upon admission and as needed, to include choice of treatment (renal replacement conference), bill of rights, patient responsibilities, and grievance procedure. These forms are explained annually, and patient signatures obtained annually, as well.
•Evaluate patients monthly and document progress notes on each.
•Provide referrals for transplant services and assist with scheduling of required tests to patients who express interest in possible transplantation and/or are referred by their physician.
•Provide assistance with patient transportation arrangements to the transplant center when needed.
•Communicate with the transplant center at minimum on a quarterly basis and ensure that the transplant list updated monthly.
•Process and satisfy requests for patients transferring to other units.
•Apply, process, and follow up on all parking permit applications.
•Satisfy requests with patients travel plans, to include dialysis clinic reservations and documents needed to secure reservations.
•Prepare letters of attendance when requested by patients or patients’ caregiver(s).
•Be available for disaster preparedness and recovery.

•Any additional duties as requested by Medical Director and/or Nurse over nursing services.
Requirements:QUALIFICATIONS AND REQUIRED SKILLS
•Must have a Master’s degree in social work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education.
•Must have a valid and current state license.
•Must have sound working knowledge of different treatment modalities.
•Must be familiar with all aspects of psychopathology.
•Must be proficient in treating different developmental stages.
•Must have served at least 2 years as a social worker and at least 1 year in a dialysis unit or transplantation program.
