Medical Social Worker Hospice

Medi Home Health and Hospice, a proud member of the Medical Services of America family, is seeking a Full-Time Medical Social Worker in Winston Salem (Davidson) NC.

· Provides social work services in accordance with the physician’s plan of treatment.

· Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.

· Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.

· Serves as a liaison with other community resources.

Job Requirements

· Must have a Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.

· Valid Driver's License.
