    • Provide home-based social services to patients dealing with depression, anxiety, poor nutrition or a lack of resources that make managing their injury or disease difficult
    • Work as part of a home health team which may include an RN, LVN, CNA, HHA, PT, OT or SLP, focused on the patient's plan of care and individual goals -
    • Monitor a patient's psycho-social condition and identify social and emotional needs -
    • Conduct patient assessments, document progress and ensure patient is moving toward goals
    • Provide counseling, community resource planning, crisis intervention and advocacy
    • Consult with family and caregivers on patient's plan of care and how to help them progressA few must-haves for Home Health/Hospice Medical Social Workers:
      • Master's degree in Social Work and active MSW license in CA
      • Minimum of 1 -years of social work experience preferred, ideally in home healthcare
      • Knowledge of state and federal home health regulations
