POSITION TITLE: Mental Health Case Coordinator
SALARY RANGE: $58,000.00 - $62,000.00 annually
REPORTS TO: Director of Mental Health Services
FLSA STATUS: Full time (40 hours), Non-Exempt
GENERAL DESCRIPTION:
The Mental Health Case Coordinator is responsible for the coordination of mental health referrals received for The CAC’s mental health intervention department as well as through the mental health clinic. This role provides administrative support, patient care, and coordination. It is crucial in facilitating effective communication among various team members, including clients, mental health providers, and Multidisciplinary Team Members (MDT).
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Receive and successfully complete training and ongoing consultation in evidence-based, trauma specific assessments
· Implement standardized assessment measures to children and their caregivers who have experienced sexual and/or physical abuse or witnessed a violent crime
· Assessment of children’s risk for suicidal ideation and safety planning
· Meet with non-offending caregivers and their children to educate and discuss mental health services provided at the CAC and mental health clinic
· Work alongside Multidisciplinary Team (MDT) members to identify and eliminate barriers to following through with treatment
· Coordination and assignment of referrals received for mental health treatment
· Adhere to mandated reporter guidelines
· Provide timely and appropriate documentation
· Receive ongoing training to ensure compliance with the NCA standards and best practices in the field of child abuse
· Assist with the development of resource materials, mental health manuals, and assist with special projects etc.
· Regularly attend Case Review, Team, and Staff meetings
· Other duties as assigned
Requirements
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to demonstrate proficiency in meeting competencies required for position; able to perform each essential duty satisfactorily with or without reasonable accommodation. EDUCATION AND EXPERIENCE:
Master’s Degree in Social Work or Psychology field, fully licensed clinician, plus two – three years’ experience in case management, intake coordination or care management with children. Experience in administration of evidence-based assessments. Need to be licensed in one of the following: LMSW, LCSW, LMHC, LCAT, or LMFT.
SKILLS, KNOWLEDGE AND ABILITIES:
· Advanced knowledge of the clinical treatment of trauma in children
· Knowledge of child abuse; child protective and law enforcement investigation procedures; and the legal reporting mandates for professionals
· Strong organizational and communication skills
· Experience working with people with diverse backgrounds
· Outstanding ability to work cooperatively with service providers and partner agencies
· Excellent computer skills and proficiency with Microsoft® Office and database software
· Respect for diversity and a commitment to providing culturally competent and inclusive services
PHYSICAL REQUIREMENTS:
· Prolonged periods of working on a computer and ability to sit during client meetings
