Job Description:

SCOPE: Serves as an expert oncology population specific clinician, patient/client advocate and assists cancer/oncology patients/families to navigate the complex health care system in an organized, effective and efficient manner. This position will assist with the standardization and efficiency of care for patients in the cancer service line throughout the continuum of patient care. The Oncology Social Work Navigator models patient-centered care and assumes leadership responsibilities with high level contributions in direct patient interaction, the department, the service line, and across the institution.

FUNCTIONS:

1. Focuses on patient/customer safety
2. Delivers personalized service using MBP Service Standards
3. Provides for exceptional patient/customer experiences
4. Intentionally communicates and engages patients/customers to ensure their needs are being met.
5. Advocates for patients and their families in overcoming healthcare system constraints by facilitating needs, education, resource provision and/or representation within the multi-disciplinary care environment

QUALIFICATIONS:

Master's degree in Social Work Required

LMSW Certification required, LCSW highly preferred

1-3 years overall social work experience required

Experience in oncology social work or a related field highly preferred
