Job Overview:
Provides direct clinical social work services to patients and their families in a multidisciplinary team approach. Completes an initial psycho-social assessment including social work treatment plan. Provides individual, family or group counseling, referrals to community agencies and entitlement programs. Ensures continuity of care for patients needs and maximizes the patient s and family s abilities to meet needs within their social and financial framework. Understands stages of growth and development to assess each patient s age specific needs to provide age specific treatment and care. Documents interventions in patient s chart.
Qualifications:
Required:
- Graduation from an accredited graduate school of Social Work with a MSW (Master s in Social Work) degree
Certifications and Licenses Required:
- State licensure required by the NJ State board of social work examiners.
- Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical course work.
Scheduling Requirements:
- Shift working; Varies with department needs
- PD
