Position: LSW or MSW licensed in OH, FT Position, M-F 8-430pm
Summary of Position: The Social Worker is responsible for the development, implementation, and management of psycho-social services to patients and families in accordance with an appropriate plan of care.
Education and Experience:
- Bachelor of Social Work Degree from a social work curriculum accredited by the Council on Social Work Education. Masters of Social Work preferred.
- Current Social Work licensure in the State of Ohio.
- Minimum of two years of experience in a clinical setting.
- Healthcare and/or Hospice experience preferred.
Special Skills and Requirements:
- Shall have a valid driver’s license.
- Excellent customer service skills.
- Detail oriented with excellent communication, documentation, and organizational skills.
- Ability to follow instructions.
- Ability to work independently without supervision.
- Travel to other sites as needed. Travel may be in inclement weather.
- Ability to lift 25 lbs of supplies or equipment.
- Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.
Responsibilities of Position:
- Completes a comprehensive initial and on-going psychosocial assessment according to accepted standards of practice and agency policy for each patient and family or as requested.
- Assess patent and family members for risk related to abuse/neglect and suicide and those with active or past substance abuse and initiate contracts as deemed appropriate.
- Provide effective counseling approaches and processes for patients and families, including therapeutic and supportive interventions, education, life review, crisis intervention, and suicide prevention when indicated.
- Assist patient and families with end-of-life care planning including knowledge of living wills, DPOA, provision of care for dependents, assessing decision capacity, and funeral planning.
- Develop and implement a plan of care for patient/family and attend IDG and care conferences as needed.
- Utilize existing community resources to assist patient/family.
- Assist members of the Interdisciplinary Team to understand the significant social and emotional factors related to the patient’s health care.
- Coordinate visits with other team members to ensure most effective meeting of patient/family needs.
- Assure continuity of care when a change in location of care is necessary (including admission to nursing home or respite setting). Facilitate nursing facility placement (PAS/RR’s) and arrange transportation.
- Provide education to employees, volunteers, and the community about services available to meet the psychosocial/economic needs of the patient.
- Assist the Bereavement Coordinator with bereavement services as requested under the direction of the Social Services Manager.
- Work evenings and weekends as needed to meet the needs of the patients and families.
- Complete all documentation as by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
