Job Summary
The social worker provides professional services to patients and families, which facilitates the use of individual, public and private resources so that optimal functioning can occur for both the patient and the family. These core services include assessment of psychosocial functioning and capacities, counseling for care planning and decision-making, social work treatment and community resource planning. Social workers collaborate with many disciplines, programs, departments and organizations in the accomplishment of these duties.
Essential Functions
- Communicates effectively. Establishes and maintains two way communication with peers, staff, physicians, leaders, and administration.
- Adapts to change in a positive, professional manner.
- Follows through on problems that may compromise patient care by using the appropriate chain of command.
- Cooperates well with peers and supports group decisions.
- Communicates appropriate information to lead staff member, supervisor, manager or director in a timely manner.
- Maintains a clear, neat and safe environment for patients and staff.
- Clearly, consistently and legibly documents all aspects of routine charting including pain management, restraints and department specific reports/documents.
- Coordinates referral process including screening as appropriate.
- Applies critical thinking and communication skills in performing job duties.
- Maintains list of community resources available to patients.
- Assist in discharge process at the request of Social Work team by setting up follow-up appointments to Outpatient resources.
- Ensures that the plans are signed by patient in a timely manner.
- Promotes effective communication and ensures continuity of care. Verbal communication with patients, families, hospital staff are always positive, accurate, supportive, and clearly understood.
- Social Worker maintains a supportive, collaborative partner-like relationship with nurses, physicians, and case management team.
- Consistently performs social service function to meet department volume and quality standard. Effectively communicates to supervisor status of current workload daily.
- Attends meeting regularly: attend daily staffing and report meetings with treatment team. Attend monthly department meetings and committee.
- Performs other duties as assigned.
- Complies with all policies and standards.
Qualifications
- Bachelor's Degree Of Arts in Social Work or Psychology required
- 2-4 years Experience preferred
Licenses and Certifications
- BCLS - Basic Life Support Current BLS or acquired within first 7 days of employment. required
- Current State License (LSW) required
