Description
The Clinical Social Worker provides individuals, families, and/or groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising patients and family caregivers, providing patient education and counseling, and making referrals for other services. The Clinical Social Worker assists patients and their loved ones with maximizing family, community and health care resources.
Responsibilities
Collaborates with other healthcare professionals and interdisciplinary team members to evaluate patients' medical or physical conditions, assess client needs, and provide information to create individual care plans
Conducts and completes comprehensive psychosocial assessments
Conducts and completes mental health and suicide assessment screenings
Facilitates clinical counseling and emotional support by utilizing and enacting clinical therapy models
Participates in and conducts discharge planning in collaboration with internal and external facilities/stakeholders as needed
Refers patient, client, and/or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education
Provides effective care and case management per plans of care
Participates in required supervision for staffing of cases and professional growth towards licensure
Assesses the capacity of the home caregiver to cope with the participant/patient's needs in the development of the plan of care
Engages clients in exploration of goals for care. Promotes and empowers clients to be an active participant in healthcare decisions
Facilitates family meetings as needed
Maintains knowledge of current regulatory, accreditation, and third-party payer guidelines as well as state and federal regulations that impact client populations
May perform other duties as assigned or requested and job specification can be modified or updated at any time
Qualifications
Required Education: Masters Degree in Social Work from a Council on Social Work Education (CSWE)-accredited institution within 6 months of hire.
Required Experience: Minimum of three (3) years in case work or counseling in a healthcare setting or social work delivery system
