The SOCIAL WORKER (LCSW) provides clinical social work services to adult and geriatric and adolecent patients and their families in the inpatient and outpatient Behavioral Health departments as needed. Psychotherapeutic techniques are utilized with individuals or groups where interpersonal relationships are examined for the purpose of achieving more adequate satisfying and productive adjustments Develops and implements age and disability appropriate psychosocial treatment plans related to personal and family adjustment and continuity of care needs. Provides information referrals and liaison activities with third party payor and community resources necessary for patients post-hospital needs."
The functions of the LCSW include making referrals for financial or other identified resource needs arrange and facilitate family/patient representative meetings with the health care team as needed assist in post-acute placement on complex discharges and facilitate continuity of care for post discharge and engagement of appropriate agencies or community resources

Responsibilities
- Primary discharge planning contact person within the adult patient social work team providing coordinated continuity-of-care communication between social workers physicians agencies third-party payers families and patients.
- Partial Hospital Program (PHP) and Intensive Outpatient Program (IOP) clinical resource providing clinical oversight as necessary for LMFT therapists in PHP/IOP.
- May schedule and facilitate team conferences/staffings, prepare patient and families for participation in conferences and/or other team coordination activities, as assigned.
- Interviews patients and families for psychosocial assessment and treatment planning needs.
- Develops and implements age and disability appropriate psychosocial treatment plans related to personal and family adjustment and continuity of care needs, through individual, family and group therapy and educational/support sessions.
- Participates in conferences/staffings and consultations with team to provide information and make recommendations for patient's treatment, discharge planning and milieu management.
- Provides information, referrals and liaison activities with third party payor and community resources necessary for patient's post hospital needs.
- Prepares timely medical record documentation of social work services.
- Requests consultation from supervisor, as needed.
- Demonstrates knowledge and skills relevant to specific program including needs of patients based on age and disability.
- Participates in marketing and public relations activities.
- Demonstrates the knowledge and skills related to the unique developmental and psychosocial needs of the patient population
- Performs other related duties as assigned or requested.
Qualifications
- Masters degree in social work from an accredited program.
- Advanced degree with field placement or job experience in hospital health mental health or related setting.
- Must demonstrate knowledge of the principles of growth and development over the life span.
- Must possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs.
- Bilingual Ability (Spanish & English) desirable.
- Management of Assaultive Behavior class required.
- Must be California State Licensed Clinical Social Worker (LCSW) required.
- Must maintain current Fire Safety cards.
Pay Range
$44.63 - $64.72 /hour
