Shift Notes: 30-39 hours per week. MWF are required workdays, while T/TH are flexible. Typically, the schedule is ~7:30/8 AM - 4/4:30 PM.
Salary Range: $30 - $40/hour (dependent upon experience)
Job Overview:
The Social Worker is responsible for providing comprehensive psychosocial support to dialysis patients, ensuring that their rights, dignity, and well-being are maintained in a safe environment. The individual will assist patients and families in coping with dialysis treatment, adjusting to lifestyle changes, and accessing necessary resources. The role involves close collaboration with the Interdisciplinary Team (IDT), ensuring proper patient care and contributing to the overall quality of the care process.
Key Responsibilities:
- Provide comprehensive psychosocial assessments for patients upon admission and annually.
- Support patients and families in adjusting to dialysis, educating them on patient rights, responsibilities, facility rules, and policies.
- Assist patients with financial aid, insurance, transportation, and home health care services, while helping improve community resources.
- Participate in patient care planning as part of the Interdisciplinary Team (IDT), ensuring seamless coordination with service providers.
- Maintain accurate patient records in compliance with facility policies and ESRD regulations.
- Contribute to the Total Quality Management (TQM) process, aiming for continuous quality improvement.
- Provide patient education and participate in facility in-service programs.
- Stay informed on developments in nephrology social work, attending relevant meetings and conferences.
- Interface with federal, state, and local agencies to secure patient resources.
- Ensure compliance with all regulatory requirements, promoting data security, confidentiality, and HIPAA standards.

Qualifications:
- Masters degree from an accredited school of social work and state licensure (or eligibility) is required.
- LCSW licensure or state-specific licensure requirements must be met.
- Minimum of one year of clinical social work experience, preferably in an ESRD setting.
- CPR certification is preferred.
