CHI St. Alexius Health is a regional health network with a tertiary hospital in Bismarck, the system also consists of critical access hospitals (CAHs) in Carrington, Dickinson, Devils Lake, Garrison, Turtle Lake, Washburn and Williston and numerous clinics and outpatient services. CHI St. Alexius Health manages four CAHs in North Dakota - Elgin, Linton, and Wishek, as well as Mobridge Regional Medical Center in Mobridge, S.D. CHI St. Alexius Health offers a comprehensive line of inpatient and outpatient medical services, including: a Level II Trauma Center, primary and specialty physician clinics, home health and hospice services, durable medical equipment services, a fitness and human performance center and ancillary services throughout western and central North Dakota.

Responsibilities
CHI St. Alexius is looking for a Part Time Social Worker to join the team!
The Social Worker is responsible to assist in the implementation, provision and coordination of social services offered to patients in the medical center. All the criteria-based duties and standards within this document will be performed according to the department and hospital wide policies, procedures, and guidelines.
What You'll Do:
- Coordinates and directs social services to ensure patient needs are met.
- Interacts professionally with patient/family and involves them in formation of Plan of Care
- Participates in psychosocial identification and planning for complex/high risk patients
- Assesses, counsels, refers and coordinates intervention relating to cases of abuse, neglect, exploitation of children, vulnerable adults, domestic violence or sexual assault.
- Identifies resources needed and works with the necessary agencies to assist patient/family with application, referrals, etc.
- Responsible for participating in multi-disciplinary discharge planning team meetings to assess and arrange discharge needs or alternate level of care.
- Completes brief assessments on complex situations and patients needing extensive social work intervention.
Qualifications
Required Education: Graduate of an accredited university with a Bachelor's Degree in Social Work. Licensed as a social worker in ND ( LSW) or in the process of obtaining license within 6 months. Until license is received will work as a social work designee in accordance with ND Board of Social Work Practice Requirements.
Experience: Minimum of 1-2 years of supervised social work working directly with individuals.
Prefer experience in a healthcare setting, specifically hospital
Pay Range
$25.27 - $33.7975 /hour
