Key Responsibilities:
- Conduct comprehensive biopsychosocial assessments and develop tailored plans to address social determinants of health.
- Provide crisis intervention, suicide risk assessments, and mental health evaluations.
- Facilitate discharge planning, connecting veterans to community resources and services such as home health care, rehabilitation, and housing.
- Lead family meetings and goals-of-care discussions to ensure holistic patient-centered support.
- Document care plans and safety measures in the medical record, ensuring compliance with VA policies.
- Identify and address instances of abuse or neglect, consulting as appropriate.
- Assist veterans with advance directives, petitions for inpatient psychiatric care, and accessing benefits.
Qualifications:
- Masters in Social Work (MSW) from an accredited institution.
- Current, full, and unrestricted LCSW licensure in any U.S. state or territory.
- Minimum of two years of recent experience in social work, preferably within healthcare or veteran services.
- Proficiency in English (spoken, written, and read).
- Commitment to VA standards of professionalism, decorum, and customer service.
Physical Requirements:
- Ability to lift and carry up to 50-100 lbs.
- Prolonged standing, walking, bending, kneeling, and reaching during patient interactions and treatments.

Hours of Operation:
Our facility operates 24/7, offering rotating shifts to meet staffing needs, including coverage during federal holidays. A typical workweek consists of 40 hours, with specific schedules determined by the Department Director and Contracting Officer's Representative (COR).
