Social Worker, MSW

Big Bend Hospice is seeking a Social Worker with a beacon of knowledge, providing education on long-term and end-of-life care, offering crisis intervention, and extending grief counseling. Dedication will be focused on meeting the psychosocial needs of those under your care, leaving a lasting impact on their lives during this crucial stage.



Schedule:



  • Monday-Friday

  • 8 am-5pm

  • On-call Rotation



Duties:


(Includes, but is not limited to)




  • Completes psychosocial assessment, initial bereavement assessment & initial plan of care on all new admissions within 5 days of admission or indicates in the record the family's request to delay the assessment.






  • Demonstrates knowledge and understanding of social, religious, and cultural aspects, counseling techniques, tasks, and dynamics of the grief process for all age groups.






  • Demonstrates a willingness and ability to manage tasks and caseloads assigned (30-35 patients), from any departmental program, in an effective, productive, and timely manner.





Responsibilities include but are not limited to:




  • Assesses & addresses a financial aspect of Pt.'s care (assisting with the completion of Medicaid Application).






  • Attends assigned Interdisciplinary Group (IDG) meetings.






  • Documents all client contacts and care plans accurately according to department and agency policy in a timely manner. This includes establishing and maintaining all legally required files according to state law, federal law, and agency policy.






  • Advocates for community resources to meet the needs of patients and families.






  • Provides crisis intervention with all age groups as appropriate.






  • Demonstrates the ability to prioritize multiple responsibilities and complete daily tasks.






  • Submits daily times sheets, as outlined by the agency that reflect expected productivity, at least 3 patient visits daily with documentation completed by 5:30 PM daily.






  • Provides on-call coverage every 6-8 weeks as assigned.






  • Demonstrates counseling skills and supports all age groups in individual and family group sessions.






  • Honors and adheres to confidentiality as detailed in departmental and agency policy and by professional ethics, and state, and federal laws.






  • Provides service to each client assigned as detailed in departmental and agency policy, and by professional ethics, state, and federal laws.




  • Communicates, collaborates, makes referrals to, and consults with other departmental counselors, members of the Interdisciplinary Group, other agency staff, and external customers with integrity and professionalism.




  • Participates in public relations activities, including public speaking, training sessions, and services. Do not accept speaking engagements, conduct training, or speak with the media without prior approval from the Director of Psychosocial Support.






  • Attends and participates in department, staff, and other meetings as requested by the agency and the Director of Psychosocial Support.






  • Makes recommendations for the improvement of programs and activities within the department.






Education and Skills Required:



  • Master's degree in social work (MSW) from a school an accredited school of Social Work (CSWE)

  • EMR and basic computer skills required

  • Demonstrated knowledge and experience in working with a diverse population

  • Ability to travel to visit the patients in facilities supporting the Care and Comfort Team).

  • Familiarity with concepts and practices of anticipatory grief, dynamics of terminal illness, and death and dying preferred.

  • Ability to manage multiple priorities and effectively handle the emotional stress of the workload.

  • Ability to remain calm, and professional and to troubleshoot, and handle patient and family crises.

  • Possess an automobile that is insured by state and/or organizational requirements and is in good working order. Must maintain a clean driving record.
