Summary The Healthcare for Homeless Veterans Social Work Program Coordinator is assigned to the Veterans Affairs (VA) Medical Center and provides services to Veterans and their significant others treated in the Homeless Veteran Program. The incumbent establishes therapeutic rapport and elicits relevant information by using interviewing skills and observation. The incumbent initiates outreach to the community, campsites, soup kitchens, local community shelters, to identify Homeless Chronically Mentally Ill (HCMI) Veterans. The target population consists of Veterans who are homeless, and who are chronically mentally ill, including substance abusers. The Outreach Social Worker provides a full range of social work services within commonly accepted standards of social work practice, which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. The Social Worker provides ongoing therapeutic intervention with the patient and significant other to assist them in dealing and coping with the psychosocial issues of mental health, aging, illness, and disability, dealing with loss and grief. The Social Worker ensures continuity of patient care in the development and implementation of the plan of care. Facilitates evaluation and treatment of Veterans in contract facilities and participates in case conferences with those Veterans. Plans will reflect continuity of care, including expected short and long-range outcomes. Social Worker collaborates with local homeless coalitions, city officials, community shelter staff, community organizations, to maximize resources for Veterans. The Social Worker has extensive knowledge of Commission on Accreditation of Rehabilitation Facilities (CARF) standards and how to apply to the Homeless Veterans program and services provided. The Social Worker provides all necessary direct and indirect care services consistent with the area of assignment. Agreed upon priorities are maintained and caseload is effectively managed while keeping coordinator informed of problem areas. The incumbent is an active participant in the treatment planning process with other disciplines. The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. The incumbent will provide consultation/education to Veterans and families on community resources, advance directives, and VA benefits. The incumbent will act as a consultant to other team/staff members. The incumbent will provide consultation/education to medical residents on social work practice and VA policy and procedure. The incumbent will act as liaison between VA and community resources, to include marketing and public relation duties as needed. The incumbent will address Advance Directives and Organ Donation in accordance with the standards of their work area. The incumbent will provide wellness and prevention education and facilitate Veteran and family support groups as appropriate. The incumbent will as appropriate, supervise the practice of social work students, or a less experienced social worker. The incumbent will coordinate treatment and communicate with the appropriate care lines or services to facilitate continuity of care for Veterans. The incumbent will maintain data and statistical compilations to comply with the Joint Commission, CARF, VA and medical center policy and procedures. The incumbent will assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness, and death. The incumbent will document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. The incumbent will coordinate appointments and provide transportation, as needed, to ensure all Mental Health performance measures are completed according to requirements. Knowledge of psychosocial treatment modalities and, under supervision, ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Establish and maintain effective working relationships with clients, staff, and representatives of community agencies. Ability to communicate effectively, both orally and in writing, with people from varied backgrounds. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: GS-12 - Social Worker Program Coordinator Experience and Education: One year of experience equivalent to the GS-11 grade level. Examples of specialized experience at the GS-11 include, but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills; maintaining patient privacy and confidentiality per policies, handbooks or directives; acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver; independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Physical Requirements: Physical demands of the work are generally minor and are generally sedentary in nature but may require occasional travel. Physical activity involves sitting, standing, walking, bending, stooping, climbing stairs, and carrying light items such as papers, books or other supplies.
