Description:
The position involves care coordination and social work duties in inpatient and emergency room settings.
Responsibilities include transition of care planning for home, home health, skilled nursing facilities, inpatient rehabilitation units, long-term acute care, and hospice.
The social worker will perform psychosocial assessments for safety planning on inpatient psychiatric units.
Assistance with difficult placements and addressing the bio-psychosocial needs of patients is required.
The social worker must provide resources and collaborate with community partners to connect patients with needed services.
The typical caseload is 15-20 patients per day, which may vary based on need.
The position requires charting in the Meditech electronic medical record system.
The work schedule is Monday to Friday, 8 am to 4:30 pm.
Weekend work may be required as needed, with holiday work depending on rotation and coverage.
The dress code is business casual.
The position does not require InterQual criteria or Milliman experience.
This role is focused on discharge planning and clinical social work.
The social worker will not require assistance from respiratory therapy or pharmacy for discharge planning.
